TCQAA Membership Application Forms

TCQAA now has separate application forms for Individual and Corporate Memberships.
TCQAA maintains a database with the information that is supplied through the use of these application forms.

For corporate memberships:
Please denote who will be the primary corporate representative, the Contact Person.
Please include at least a valid email address for each member.
Please make payment with a check. TCQAA does not have the facilities for accepting payment via credit card.

For renewing Members, a membership application may not be necessary, unless you have changes to your contact information that you want to communicate to TCQAA.  Payment with your name, (home or work) email address, and (if you have it) your Member ID, which is on your TCQAA Name Badge/Card, is sufficient.

Print*, fill-out, and bring the application to the next TCQAA meeting,  OR  mail with a check to:

*Also, you may "pre-apply" and "pre-renew" by filling out a copy of the Word document and emailing it to the Membership Committee.  Or, put the pertinent information directly into an email.
